Welcome to ONEai Health (ONEAi/One) (the application), which is published by ONEai Health. This Privacy Policy applies to the ONEai Health application, all of which are provided by ONEai at any time and from time to time in its sole discretion. ONEai Health may change, suspend or discontinue any or all of the ONEai Health application at any time.
This Privacy Policy applies to information collected by Apple Health Kit about your health and wellness data, and you agree that your use of the ONEai Health is subject to this Privacy Policy.
If you do not agree with this Privacy Policy in its entirety, please do not use ONEai Health.
Information Collected by ONEai Health
ONEai Health (ONEAi/One) does not collect any personally identifiable information from you except your phone number for the sole purpose of account authentication, and the information you directly authorize Apple Health Kit to share with ONEai Health in providing health and wellness data points. ONEai Health will not sell, share or rent personal information that is collected in the application in anyway other than as disclosed in this Privacy Policy.
Compliance with Laws
ONEai Health (ONEAi/One) will not disclose personal information to third parties other than as provided in this Privacy Policy, except when required to do so by law such as to comply with a subpoena or similar legal process, or you have otherwise consented to additional use or disclosure of information.
Data Retention
We retain information for different periods depending on the type of information, how it is used, and legal or contractual requirements. We may retain information longer than the periods described below when required to comply with law, respond to legal process, support audits, prevent fraud or security incidents, or resolve disputes.
Account and contact information (for example, phone number, email address, and account identifiers):
Retained while your account is active. If you request deletion, we delete or de-identify this information within 30 days after verifying your request, unless we must retain it for legal, security, or fraud-prevention reasons.
Device/app technical data (for example, device identifiers, crash logs, and security logs):
Generally retained for up to 24 months, unless a longer period is needed for security, fraud prevention, or legal compliance.
Health and wellness data (including Apple HealthKit and connected-device data):
- If you use ONEai as part of a provider-prescribed program, some health data may be treated as protected health information and retained in accordance with your provider’s instructions and the longest applicable retention requirement under law and contract. HIPAA does not set a universal medical record retention period; state laws generally govern how long medical records must be kept.
- Certain regulated entities (for example, hospitals and nursing facilities) may be subject to minimum medical record retention requirements (for example, at least 5 years in some federal programs), which can affect retention expectations for clinical program records.
HIPAA compliance documentation:
Where applicable, we retain HIPAA-required administrative documentation (such as policies and procedures) for at least 6 years from the date created or the date last in effect, whichever is later.
Biometric data (where subject to biometric privacy laws):
We maintain a retention and destruction schedule and destroy biometric identifiers/information when the purpose for collection has been satisfied or within 3 years of your last interaction with us, whichever occurs first, unless a longer retention period is required by law.
Geolocation for Care Management
By using the app, consent to ONEai collecting precise geolocation from my mobile device (including GPS, Wi-Fi, and cellular-based location) to help manage my healthcare.
I understand:
- What: precise location data from my device.
- Why: only for care coordination — recognizing when I’m at home, at a healthcare facility, or away from my usual care setting, and enabling timely outreach, follow-up, and transition-of-care support. My location will not be used for advertising, marketing, unrelated profiling, or sale.
- Who receives it: my authorized care team, ONEai as a HIPAA Business Associate, and authorized healthcare partners involved in my care — otherwise only as permitted or required by HIPAA and applicable state law.
- Protection: geolocation is retained only as long as needed for these purposes and protected under the same safeguards as my other health information.
- Withdrawal: I may withdraw consent at any time by turning off location permissions on my device, disabling location sharing in the ONEai app, or notifying my provider or ONEai in writing. Withdrawal applies going forward.
- Choice: consent is optional; declining may limit location-dependent features but will not affect my eligibility for CCI services. Geolocation may be delayed or inaccurate and does not replace contacting my provider or calling 911 in an emergency.
By continuing use of the ONEai Health app, I give my affirmative, informed consent to the collection, use, and sharing of my geolocation as described above.
De-identified / anonymized data:
We may retain de-identified or anonymized data indefinitely for analytics, research, quality, and product improvement because it is not intended to identify you.
How to Request Deletion of Your Account and Data
If you created an account in the ONEai Health app, you may request deletion of your account and associated data in the following ways:
- In the app: Go to [Settings > Account > Delete Account] and follow the prompts.
- On the web (no app required): Submit a request at https://oneaihealth.com/delete-my-data/.
- By email: Email help@oneaihealth.com with the subject line “Account Deletion Request” and include the phone number or email associated with your account.
Verification and Timing
To protect you, we may ask you to verify your identity before processing a deletion request. After verification, we aim to complete deletion requests within 30 days, unless a longer period is required or permitted by law (for example, where backup/archival systems are involved).
What Deletion Means
When you request account deletion, we will delete or de-identify data associated with your account that we are not required or permitted to retain. We may retain certain information for legal, regulatory, security, fraud-prevention, billing, audit, or dispute-resolution reasons, as described in the Data Retention section.
Changes to This Privacy Policy
This Privacy Policy is effective as of 04/18/2026 and will remain in effect except with respect to any changes in its provisions in the future, which will be in effect immediately after being posted on this page. We reserve the right to update or change our Privacy Policy at any time and you should check this Privacy Policy periodically. Your continued use of ONEai Health after we post any modifications to the Privacy Policy on this page will constitute your acknowledgement of the modifications and your consent to abide and be bound by the modified Privacy Policy.
